There has been a lot of discussion here regarding the initials, with 
most conclusions both here and elsewhere (including the Harry Potter 
Lexicon, which JKR herself has acknowledged as a reputable source of 
HP info) that RAB is indeed Regulus Black.

As far as LV personally killing Regulus, Sirius actually said the 
opposite in OOtP (US hardcover, page 112):

"No, he was murdered by Voldemort. Or on Voldemort's orders, more 
likely, I doubt Regulus was ever important enough to be killed by 
Voldemort in person."
