Are you saying that the UV can "see" through the disguises? That DD!
Snape is about to die from the UV unless he kills Snape!DD? 

The third clause of the Vow required Snape "should it prove necessary 

 if it seems Draco will fail [...] carry out the deed that the Dark 
Lord has ordered Draco to perform?"

But Draco does not seem to be about to fail the /deed that the Dark 
Lord has ordered/--namely killing Dumbledore.  Rather he is about to 
fail in the attempt to kill Snape (as DD!Snape).  So it seems to me 
that the Vow would not even kick in.
