Here's my theory, for what it's worth. The pustules are ongoing because the
effects of the breaking of that trust is ongoing. In order words, as long as
the rest of the DA feel the effects of being discovered, Marietta will feel
the effects of the hex. It may be that the bubotuber pus antidote works, but
that the pustules are reforming anyway. I think Hermione was vengeful enough
to not have it be a one-time breakout that would disappear, but have it be
something like a Scarlet Letter that would announce for a very long time
Marietta's transgression to her fellow classmates. If you remove the hex,
then you remove the effects, and that is more of a job for a competent DADA
teacher instead of a school nurse.
