I think the most likely explanation is actually Fawkes transporting him. We
know that you can't apparatus in or out of Hogwarts and I don't really see
the Headmaster would be an exception. As far as I understand it, you cant
just lift it for one person. We do know however that Dumbledore travels by
phoenix out of Hogwarts - in OOTP for example he escapes by this method. 

Portkeys also require a Ministry license, something Dumbeldore often
disregards, but given the secrecy of the mission. although this would
explain why the Ministry knew he was leaving.

Moreover the phoenix is a loyal pet and would recognise the problem and
(conjecture) possibly try to heal it with tears?
