I don't think anyone was saved by luck or coincidence. Riddle knew that if anyone was killed the school would be closed and the diary would be useless. Since Riddle's goal was to entice Harry to discover and enter the chamber, it would have been counterproductive to kill Hermione, or to take the slip of paper with its clue from her hand.
