It's in Chamber of Secrets:

"'Oh, are you starting at Hogwarts this year?' Harry asked Ginny.

"She nodded, blushing to the roots of her flaming hair, and put her
elbow in the butter dish. Fortunately, no one saw this except
Harry...." (CoS Am. ed. 44)

I don't know the page number for the UK edition, but it's in chapter
4, "At Flourish and Blotts," a few paragraphs after the list of books
for second-years.

Nothing to do with OoP, though I seem to recall a similar scene
involving a gravy boat in a later book, possibly OoP, while they're
staying at 12 GP. I didn't check, though, since I was looking
specifically for the butter dish.

FWIW,JKR seems to associate girls and young women who are in love (or
in the throes of a major crush) with clumsiness. We see the same thing
in Tonks, who only loses her clumsiness when she's depressed, at which
point, she's able to jump from a moving train and land on her feet
(HBP). Depression has its advantages, apparently. <eg>
