I can't actually remember whether it goes by all your rules, but I 
have a great favourite among fanfics, which is Ezzie's Destiny of 
Souls and the sequel, A Mortal Enemy (both are novel-length fics, 
both basically finished - AME lacks a last chapter but really doesn't 
need to be good). They can be found on fanfiction.net, search for 
Ezzie. 
They are Snape/OC, but actually as the story goes on we get to see a 
lot of Harry&Co as well, and geez, it has a so-called PLOT. A very 
good one, too, I think.
