I don't like blaming Lupin for this as I can't see how (as explained earlier in your post)
It is actually something else that bugs me, how could Lupin have had the potion with him in the first place (as pointed out by you) to have forgotten to take it? And how can Snape have known that he forgot?
This is what bugs me:
When Snape takes off the cloak he says that the reason he went to Lupin's office (and so saw the map) was because "You (Lupin) forgot to take your potion tonight, so I took a gobletful along." 
Well how would Snape know (without first visiting Lupin's office) that he had forgotten to take his potion?
We can presume that, normally, the following would happen (from when Harry is drinking tea with Lupin):
Snape brings Lupin a goblet of potion. (as he did when Harry was there)
Lupin drinks potion (once Snape has left, as he did with Harry)

Well in that case how would Snape know he had forgotten to take it without going to Lupin's office to check?

The only way that Lupin could have forgotten to take it and for Snape to know, is if Snape never took it to the office, otherwise Snape would have taken it to the office, Lupin would have left it to go chasing after the others and Snape would have never known that Lupin hadn't taken it until he discovers (along with everyone else at the school) that Lupin is running wild in the Forbidden Forrest.

So does Lupin have to request Snape to bring the potion along or what? That just never really made sense to me so (even though it is really highly unimportant) I just thought i would ask you lot.
