I think that Dumbledore assigned Snape (whom I see as somewhat of a 
security chief in Hogwarts) not only to brew the Wolfsbane potion for 
Lupin, but to make sure that Lupin does take it.  It's an 
understandable security measure--just imagine that Lupin gets too sick 
or injured or otherwise physically incapacitated and unable to grab 
the goblet, or, say, too distracted (as on the Shrieking Shack night) 
and forgets about the potion...

That's why Snape makes his business to check if Lupin forgot about the 
potion and takes trouble to deliver it in person, IMO.

On the night of Sirius' escape both men forget about their duty amidst 
all the excitement and later both blame themselves for the werewolf 
situation getting out of control... and rightly so. *grin*
