But do we know if James and Sirius were caught? Hagrid made a comment
in PoA that Fred and George could give James and Sirius the run for
their money, but Fred and George are sometimes caught (throwing
snowballs at Quirrell's turban in SS/PS, dungbombs, etc.). With the
exception of the Snape incident, I get the feeling that James and
Sirius had the reputation of being mischeivous but not to the extent
of the Weasley Twins.

Actually, I don't see Harry as a prefect. Dumbledore and McGonagall
know Harry (along with Ron and Hermione) has other
responsibilities/concerns even more pressing than supervising
students. With Voldemort on the loose, I think it would be rather
irresponsible of Dumbledore to give extra responsibilites (and their
concommitant anxieties) to them. Also, I have a feeling that
Dumbledore knows Harry is destined to defeat Voldemort for good. I
really don't see how making him a prefect will help him hone his
skills. If you ask me, Dumbledore should start training Harry for his
final battle (if there ever is one) rather than making him a prefect.
