Last time this came up I checked it out.  It's clear from Lupin's 
discovery of Peter that Hagrid's hut is off the edge of the map.  So 
is the Shrieking Shack (Hogsmeade).  On the two occasions when Lupin 
and Snape looked at the map, therefore, only one of each would be 
visible. Nobody else looked. (I like the idea that Dumbledore went 
into Lupin's office, saw two of each, and realised he had to tell 
them to use the time turner, but it can't be: in GOF he didn't know 
about the map.)

various theories have been put forward.  Mine is that F&G saw PP many 
times, and either couldn't work it out or thought of it as 
Scabbers' 'real name'.  After all, the map shows Mrs Norris (what 
does that say about her?).  They were in no position to inquire, as 
they weren't supposed to have the map.
