Terrific observations, Jason. I must admit that this theory hadn't
ever occurred to me. If I can pick at it a bit, though, when Harry
tells Dumbledore what happened in the graveyard, he doesn't mention
that Wormtail cut off his hand. A rather large omission, no? Maybe
JKR just doesn't tell us that Harry mentions this. Also, if the use
of Wormtail's hand for Voldemort's re-birth undermines the protection
Harry has, then why can Voldemort touch Harry now?

Hmmm. You may have a point that Voldemort knows that Snape is
working as a double agent. The scene you cite in PS/SS, coupled with
Dumbledore telling a huge room full of people in the Pensieve scene
that Snape is a spy, suggests that Voldemort has figured out Snape is
the traitor and plans to kill him on sight.
So (as you ask), where *did* Snape go that night? It can't be to
return to Voldemort, because Snape wouldn't have come back in one
piece.
Brace yourself for a totally wacky theory. I think Snape the Potions
Master went off and drank some polyjuice potion to become Crouch Jr.
I think I recall that polyjuice potion takes a while to brew. So
Crouch/Moody must have had a cauldron simmering somewhere, ready to
go once he adds some hair to it. Snape would have found that batch
of potion, snipped some of Crouch Jr.'s hair, and gone off to join
Voldemort, not as Snape, but as Crouch Jr. Remember, Voldemort
doesn't know Crouch Jr. has been subdued, and he plans to honor
Crouch Jr., the faithful DE, beyond all others. Snape knows
everything he needs to know to impersonate Crouch Jr. because he was
conveniently present for Crouch Jr.'s interrogation.
Also, we are told that Dumbledore and Snape seem awfully nervous
about whatever it is that Snape has to do. If Snape is likely to be
welcomed back into the ranks of the DEs as Snape, there's no reason
to be particularly nervous. But if he has to use polyjuice potion,
and if he can't find a way to take it hourly, he will be discovered
and killed.
The Snape-as-Crouch-Jr. thing has lots of exciting potential for
future stories. We know that Crouch Jr. will be honored over all
over DEs, so Snape will be in a position to do some serious spying.
He'll know everything! For instance, maybe Voldemort will capture
Karkarov and assign Snape the task of "disciplining" Karkarov with
Crucio or even Avada Kedavra. The internal conflict possibilities
are delicious.
Having said that, I am listening to GoF also, and I heard something
interesting in it for the first time. When Snape shows Fudge his
dark mark, he says: "Every Death Eater had the sign burned into him
by the Dark Lord. It was a means of distinguishing one another, and
his means of summoning us to him." What does Snape mean when he says
the dark mark was a means of distinguishing one DE from another? Are
all dark mark tattoos different? If Voldemort wants to summon just
one DE, and he touches Wormtail's mark, do all of the dark marks
burn, or just the mark of the DE Voldemort needs right then? I have
no idea. Any ideas?
