House Ghosts and The Grey Lady

We know that the Bloody Barron belongs to Syltherin, Nearly Headless 
Nick to Gryffindor and Fat Friar to Hufflepuff but I cannot the 
references in Book 1 that introduce the Grey Lady and Ravenclaw's 
ghost. Can anyone tell me the page numbers or chapters where she is 
mentioned? I can't find them and all four ghosts are pictured in the 
Vanity Fair issue that I just got. Thanks, Sarah
