I think I've read about this before, and IIRC, she's not specifically 
named as either the Grey Lady or the Ravenclaw ghost - which 
makes it even more fun to find her in the book.
If I'm right, in Ch. 12, The Mirror of Erised - page 154 in the 
Raincoast (Canadian) edition -  Harry and Ron are wandering 
around in the dark, under the invisibility cloak, trying to re-locate 
the classroom where Harry found the Mirror the first time, and 
they pass "the ghost of a tall witch gliding in the opposite 
direction". That's her.
