No book handy, but the Slytherin common room entrance was through a
blank wall (polyjuice chapter CoS) not a portrait.

McGonagall does NOT set them for Gryffindor House.  No book handy, but
Sir Cadogan spent his time challenging people to duels and thinking up
ridiculously difficult passwords which he changed twice a day.  (It was
a list of these Crookshanks stole from Neville.)  Since, Sir C. does,
it follows that the Fat Lady also makes up her own passwords.  (Do you
really think McGonagall or Percy would come up with 'fairy lights' and
'flibbertygibbet'?)

Even if prefect set it, I don't believe he/she would choose a password
like 'pure-blood' unless he/she knew for sure the Head of House would
be okay with it.
