I've always thought that it's because the hero's best friend being 
killed is one of the biggest cliches in literature.  JKR even 
alludes to this in one of her interviews when she is discussing the 
fact that this is one of the biggest worries expressed by the 
children who are fans of the book.  I can't recall which interview 
she says this in although it's one of the ones on Aberforth's goat's 
page, but even little kids have seen enough examples of this in 
books, movies and tv, to immediately see Ron as a possible candidate 
for this overused plot device.
I can't see JKR killing off Hermione just because no one expects 
it.  She has her own story to tell, and I really don't think she 
cares what the readers expect her to do or not do.  I also think 
she's too good a writer to play the "kill the best friend" card just 
to provide Harry with additional rage-motivation to take out Voldy 
(although bumping off Hagrid, Sirius, or Lupin could have that same 
effect).  The foreshadowing of doom for Ron and Harry is IMO a big 
red herring, so that we don't get too comfortable while waiting for 
her to finish the series.
