I LOVE your theory about the "Order of the Phoenix" title.  It makes 
sense to me.  The title could even have many meanings. JKR has a 
tendency to revisit her favorite subjects (like polyjuice potion) and 
I can't believe she'll let the priori incantatem theory rest forever 
(do you think that if Harry was holding Fawkes, he could do more 
powerful magic than with his wand?). 

However, I'm not sure I believe that Dumbledore will die in book 5.  
I think his death will be closer to the end of the series.  Harry may 
have come of age, but he's still got a lot to learn.  I also wonder 
who would explain the story at the end of the book if Dumbledore 
dies.  In each of the books so far, Dumbledore has exposed the 
importance of the lessons in the book at the end.  So far in this 
series, we have learned almost nothing about Harry's parents or 
heritage.  I find it hard to imagine that Dumbledore will die before 
passing on a lot more information in this department.

I agree with a recent article I read (can't remember author) that 
states that the death may be Molly Weasly.  If not her than Hagrid.
