Well, Tom Riddle had renamed himself 'Voldemort' when he was still at 
school. In CoS: "You see?" he whispered. "It was a name I was already 
using at Hogwarts, to my most intimate friends only, of course". (ch. 
"The Heir of Slytherin")
I doubt that Riddle would have "fashioned" himself this specific name 
if he hadn't already set his mind on achieving immortality.
