My 2 cents:
Maybe Dumbledore was trying to get into LV's "emotional" side - 
or appeal to the deeper person instead of the monster that he 
has become.

Another possibility- calling him Tom could be considered an 
insult?  Instead of calling him Voldemort like I'm sure he'd like to 
be called, Dumbledore is then talking down to him?  Just a 
thought...
