Now me:

Well,  the picture in my mind was of James hearing about it from inside the 
castle and then running out just in the nick of time.  You know, he was 
calmly reading by the fireside in the commons room when:

Peter, suddenly turning from rat to human bursts in.
James: Peter! are you mad, why did you just transform for???? *in a hushed 
voice* You could have been found out!
Peter is too worked up to reply to that, but manages to stammer out, 
"Sirius... He told... Snape's going to the shack... he can get past the 
willow...
Peter hasn't got time to finish before James bolts out the portrait-hole and 
down the stairs.

(As you can see, i do like the idea of Peter telling James, it fits with 
their roles.)

Anyway, this is why i think my scenario fits both your questions.  James 
would have wanted to get to snape and lupin as fast as possible.  Prongs can 
run faster than James can, so if he was on the grounds, but not within sight 
of severus, it would make more sense for him to be prongs.  Likewise, if he 
was in the shack, or on his way, one would hope that he would be prongs 
already.  However, if he was inside, he couldn't be Prongs.  Not only is a 
stag an odd thing to see in school,  but, i have a suspicion that deer might 
be like cows: their muscles can't move to let them go down stairs. (which 
makes me wonder, how *does* a stag move about in a shack? is he small enough 
to get through the doors? wouldn't his antlers get stuck??)

Although, James & co. were very cocky about how 'tame' Lupin had become.  He 
may very well have been in wizard form in the shack or the tunnel, thinking 
that lupin would never hurt him.
