In my opinion, JKR makes it pretty clear that Draco is
faking from early on.

Diane (dianasdolls) posted an excellent recap of the canon supporting
this earlier today.

There's no doubt in my mind that Draco was basically fine
right after treatment, and used his injury to the fullest,
not only to get back at Hargrid, but also to taunt Harry
and Ron ( basically admitting it to them outright).
