I agree that the eyes might have been switched in some way.  If 
Lily's dying were enough to protect Harry, then why wasn't James 
dying good enough?  (When Harry was having the flashbacks of his 
parents dying, it seemed to me that James died fighting of Vol. 
while giving Lily and Harry time to escape.)  Maybe while Vol. 
attention was diverted by James, Lily managed to have enough time to 
cast the protective spell, and switching the eyes had something to 
do with it.  The eyes are the "window to the soul", and maybe 
switching the eyes caused the Avada Kadavra spell to go horribly 
wrong, maybe killing Lily instead, and at the same time causing the 
spell to bounce off Harry, and hit Vol? 

Lily's wand was supposed to be good at charms.  Is the switching 
spell a charm, or a transfiguration spell?  Maybe the eyes aren't 
really switched (doubt it), but Lily put a type of protective charm 
on the eyes, and in the process changing the color of them to match 
hers?  So many possibilities...
