The pain would only come when Voldemort was facing him - & quite probably, 
only when Voldiekins was strong enough (i.e., when he'd just fed); it's quite 
possible that Quirrell never turned his back toward Harry in class when Voldie 
was strong enough to affect the scar.

(One would assume he'd also have to be awake...difficult, in one of 
Quirrell's lectures, IMHO, if Umbridge approved of him...) <g>
