Sirius said that he wasn't sure how he maintained
his sanity, and assumed it was because he turned
into a dog.  This is not necessarily true.

My thought is that he kept his sanity because he 
had no happy thoughts for the dementers to feed 
on.  James (his best friend) and Lily were dead, 
their son (his godson) orphaned, and the guilty 
Peter Pettigrew was on the loose.  And to top it 
off, he was imprisoned for the crime.  He felt 
complete guilt and no happiness.

The Death Eaters may have kept their sanity 
similarly.  Their leader (to whom they had given
their complete loyaly) was destroyed.  They were
his known followers, so their happiness depended 
on his success.  They may not have felt guilt over
his collapse, but they longed for revenge.  No
happiness comes from that.
