My Thoughts:

It seems to me that the MoM has "ways" of knowing when (and who) 
commits magical acts.  Could they not also know where these people 
are?  When Harry produces magic out of school, the MoM respond 
almost immediately.  I don't think it would be a far reach for them 
to figure out the DD would have Harry under some sort of guard (even 
if they think he's just an old guy who's loosing it) and begin 
to "monitor" for magic in the vicinity.  Then, Umbridge would have 
known for about a month the comings and goings of various "guards" 
of Harry.  She searched out the weak link and found a suitable lure 
for him.  I don't currently think there is a spy in the order, but 
don't know that one won't exist later.  Perhaps Kreacher wasn't 
telling DD everything afterall?  

On a somewhat separate note, now that Sirius is gone, is Kreacher 
still bound to the house and his former promises?  What happens to a 
house elf when the last survivor of a family line has passed?
