No, we don't know for sure, but we can certainly suspect with relative certainty. In CoS, when Harry sees our beloved bird burst into a burning blaze (ooo! alliteration!), Dumbledore remarks that (paraphrasing) "It's a shame you had to see him on a Burning Day." Sounds like this happens more than once every hundred years. After, Dumbledore is a bit over 100 (140, I think?) so this would be, maybe, the second time this has happened to Fawkes in Dumbledore's longest possible possession. I'd expect Dumbledore to be a bit more disappointed that he had missed it. Furthermore, Fawkes was "killsurrected" in the MoM, but was that the first time? Maybe. You're right that we don't know the length of a phoenix's life cycle; it could be month to a years, but it is, I'd imagine, regular. The Burning Day is the day Fawkes dies and is reborn, so, as long as Fawkes doesn't get AK'ed, he'd probably have about the same length of life (lives). But there we are.
