I assume the question mark on lable was put there *before* Harry was
born (as the prophesy was made before then). They wouldn't have know
who it was about before then would they? After LV attacked Harry, it
may have seemed obvious that it was Harry the prophesy was about, so
his name would have been added *after* the original lable, but the
question mark could have remained (it doesn't seem that quills have
erasers).
