Harry, for example could have used the mirror to try to contact 
Sirius. I think Harry is partly to blame - partly. As are also at 
least Snape, Dumbledore, Lupin and even Sirius himself. But to blame 
all these characters is IMO, like to blame Dobby or Hagrid for 
Voldemort's return, because they helped Harry in the TWT. Sirius 
death' was IMO the result off several mistakes and several incidents, 
and than it happened. And if anyone is really responsible, than I 
would say it sure is Bellatrix Lestrange.
