Yes, the veil means death. It is meant literally. The stunning charm 
didn't kill Sirius, it was his falling behind the veil.

I think it is Sirius, because the death of Sirius is the worst that 
could have happened to Harry. Yes, there is Hagrid, and Mrs Weasley 
and Ron and Hermione and and and. But Sirius was his godfather, they 
were never able to say goodbye, in fact, they were never really able 
to get to know each other, and this is horrible for Harry. Also, it 
was a truly shocking scene for the audience, that a character with 
such a horrible life, died such a senseless death.

Voldemort didn't know what exactly the second part of the Prophecy 
was. Neither did his Death Eaters and Harry and his friends didn't 
know anything at all. All of them might have thought, it is more 
important, than it actually is (or at least currently seems to be). 
The prophecy also makes clear, that Dumbledore can't kill Voldemort.  

@ Lilys Eyes: I know JKR said something about Lily's eyes being 
important, but I never read, that she said we would find out already 
in book 5. Maybe it is just meant the way, that the eyes are the 
mirror to our souls, and that Harry is characterwise more like Lily, 
than like James.
