OOP: What killed Character X and questions

Sirius was low on my list of people I thought would die in OoP. I
wasn't sure he'd make it through the series, but I figured he'd be
around for a while for a number of reason.
1) The Parting of Ways chapter in GoF - Snape and Sirius shake hands,
however unwillingly. I took that as a clue that they'd end up
working together at least part of them time in OoP and would have to
find a way to overcome their differences for the greater good.
2) The Godfather thing - I'd also hoped for a greater personal face-
to-face connection between Harry and Sirius, as a way for both of
them to give and get some much needed emotional support. Harry at
some point would become more independent (and would have to, given
his particular function in life as potential Vmort Exterminator), but
again, I thought that would come in Book 6 or 7.
In OoP, I started to get a bad feeling about Sirius' survival from
the very first time we saw him. He was portrayed as trapped, useless
to the cause (or at least feeling useless). Mention is made several
times about his past recklessness or rashness or irresponsibility.
JKR pretty much made his role to have no role in the workings of the
Order. to sit on the sidelines. And that just made me think that he
would of course do something spur-of-the-moment and get himself
killed.
Plus, he struck me as much more emotionally volatile than he was in
GoF, and his emotions were often so negative that I developed an
increasing sense of foreboding. Even so, when I read the death scene
I didn't want to believe it.
