No idea what Bella hit Sirius with but it didn't instantly kill him, 
Harry sees a live expression on sirius' face as he falls through the 
veil.
Shacklebolt was not AK'ed. None of the DE's got off a successful AK 
and I suspect that they were not powerful enough to perform the curse 
in battle. I am assuming this, because the one time the AK was used 
in the MOM, the DE who used it, had a space of time to "work up to 
it" so to speak before attempting it. 
Voldemort on the other hand performed the AK in full fledged battle, 
which is scary, don't you think.
