Repeated Names in Harry Potter Books

Is Professor Trelawney's great grandmother's last name ever 
mentioned or is it just an assumption that it is Trelawney? Because 
it would seem a big coincidence that her ancestor, described as a 
well known seer if I remember correctly, would share the same name 
that another well known seer (unless she was named after said seer, 
or the other way round).

If the last name isn't actually mentioned, then I think Trelawney's 
great great grandmother's Cassandra *is* Cassandra Vablatsky. Any 
daughter in the lineage between Sibyll and Cassandra would be enough 
to explain the last name difference (or Sibyll Vablatsky could have 
married a Trelawney).

Another possibility, if the name Trelawney *is* mentioned for 
Sibyll's ancestor: Cassandra Vablatsky could be actually Cassandra 
Trelawney, and could write under her maiden name (Vablatsky) or 
something of the like. She probably would have been known as a seer 
in the WW under the name Vablatsky way before her marriage, and 
might have already published books under that name. Then she kept 
using the same writing name.
