The textbook is almost certainly by Trelawney's great-great-
grandmother, which is probably why she chose it. If the relationship
is in the maternal line, Trelawney could have a different last name
from her great-great-grandmother without being married herself. I
don't think the name Cassandra is a coincidence (though I hope
Cassandra V. wasn't cursed like the original Cassandra by having her
prophecies disbelieved).

Actually, we're given Percy's full name in the hearing chapter of OoP:
Percy Ignatius Weasley. I think the Percival in Dumbledore's name
relates to the Grail legend (Parsifal, etc.) whereas Percy is the last
name of a number of aristocrats in English history, including the
Earls of Northumberland. Percy Bysshe Shelley's first name was
intended to suggest that connection. In Percy Weasley's case, it may
suggest his own grandiose ambitions.
