Actually, yes. I think all these are quite possible -- and may have happened
in the past, which might be why earlier tournaments were so dangerous. But
it's probably not well know, and if responsible people like Dumbledore have
their way, never will be.
Also, the things which the Goblet *would* have rejected, such as a second
candidate from the same school, were dealt with -- as Dumbledore suspected
and Crouch Jr. confirmed (I think) -- by Crouch/Moody's confusing charm.
