Snape's goading started those failures, so yes, in my book, Snape is 
mostly to blame for them. 
Not to humiliate Harry at first class was Snape's responsibility 
first and foremost. Teach Harry occlumency properly was Snape's 
responsibility first and foremost.
There are times when Harry knows that he is in the right and what 
Snape says  ( about Harry's parents, when Harry wants to warn 
Dumbledore about Bary Sr, etc.) still humilating and degrating to 
him.

Harry doesn't see any reason to break a sweat in the subjects he 
doesn't care about, and he didn't care about 
potions.

Yes,Harry does not care much for Potions and Snape can be very proud 
of himself for starting it, doesn't he?
