Kreacher did have to obey.
Sirius said "Out!" which Kreacher interpreted as an order to leave.
But Kreacher could lie to his master, provided he punished himself afterwards.
Kreacher's bandaged fingers show that he was lying when he claimed to have
been in the attic.
What I wonder is whether Dumbledore offered Sirius the option of having
Kreacher work at Hogwarts. If so, did Sirius turn it down?
