Wouldn't driving someone to insanity be worse then killing them, in a
sense?  They would be in a prolonged state of suffering.  If she is
willing to do that then surely it is only a matter of time before she
takes the next step to killing.

I don't think Sirius going through the veil is just a pure accident.
If it was there are hundreds of ways J.K could have had him die
without him going through the veil.
