Some thoughts,

1. Voldemort's worst fear is death.  IMR, in the duel scene in the MOM 
in OOP Dumbledore said that Voldemort's failure to understand that 
there are worse things than death is his greatest weakness.  The idea 
of willingly giving one's life for another is completely foreign to 
him.  Certainly, he is well aware of the fact that his own mother was 
not even willing to stay alive for him when he was a baby.

2.  According to Dumbledore Voldemort has never understood, and has 
therefore always underestimated, the power of love.  He sure 
underestimated it in GH, to his detriment.

3. It never happened before.
