Because it is not someone's words that matter most to me, if I am
trying to determine what matters most to them. People can lie, people
can fail to understand (or admit) their own motivations to themselves
for any number of reasons. It is their actions that are most telling.
Sirius's actions in GoF and OotP speak for themselves. He does care
about Harry, and his well-being is a big motivation for Sirius's
actions. But the evidence of his actions in PoA indicates pretty
clearly to me that revenge was a big motivating factor for Sirius as
well, in that book.
I'm not sure what Carol meant by bringing Harry and Snape into her
post, but I see Snape as a prime example of this idea in action in
reverse. Based on his words and demeanor, he could not care less
whether Harry lives or dies. Based on his actions...totally different
picture.
