JKR has had these stories in her head for 17 years. It would be hard for her to see the gaps since she knows the backstories and has been so close to the project for so long. I think the fault lies with the cadre of continuity editors and others at publishing houses on two continents who should be picking up on these gaps before the books go to press. Appears they're not earning their salaries.
