Good questions all; I'm only going to comment where I have something
different to say than the others. 

On this one I assumed the MoM recovered the body and that's how
Umbridge got the eye, which must have seemed too useful to dispose of
along with the rest of him.

I think we're supposed to see them not so much as stupid but cruel and
power mad. Rather than using the most expedient or clever strategies,
they automatically resort to deadly force or torture.

I agree that this doesn't fit the prophecy; the fact of the matter was
that neither could DIE while the other lived. I can only surmise that,
as DD explained to Harry in OotP, most prophecies go unfulfilled due
to the free will of the people involved. The prophecy seems to have
played the role of setting the ball rolling, and motivating the
players' actions, but it didn't predict the outcome.
