Mmmm... Let me see... :-) If DD had called her to his office, may be 
the significance of sword of Gryffindor made her notice it even as 
she had just entered the room - even before DD started to explain why 
he had called there after which, I suppose, she might not have been 
able to notice much. Yeah, possible, I guess. :-)

Yes, there is a possibility. Since we don't have a day-to-day account 
of what happened and most of it is Harry's POV, it is possible that 
this kind of information is left to the imagination / deduction of 
the reader.

LOL. May be it didn't and hence, all the chaos. Otherwise, at least 
DD would have noticed, given the effort he had put in to determine 
the locations of the horcruxes.
