I think that Trelawney's second prediction has indeed come to pass.  
Because of the Bone, Flesh and Blood spell, LV is ressurected and can 
now touch Harry.  What will now happen is not relevant to the rising 
again of Lord Voldemort, truly more terrible than before, which has 
now transpired.
