In Sirius's description of Crouch and in Dumbledore's memories in the 
Pensieve, we find out that Crouch is a fanatic. Well intentioned (probably), 
but not completely sane when dealing with Death Eaters.

>From there, if his colleagues know him as an eccentric, they aren't going to 
be surprised if it branches over into other things as well.

Also, Crouch is stressed from preparing for the Quidditch Cup and the 
Triwizard Tournament, so no one will be surprised if he seems tired -- 
they'll think it's a one time thing. Remember, most of the time Percy and 
Crouch are together is at work, so nobody except Percy is going to know 
that/if Crouch calls him by the wrong name on a regular basis.
