I would agree with this if the Yule Ball never happened at Hogwarts.  However, she did attend the Yule Ball.  This is a pivotal (sp?) point for her in the books.  Here she has a famous Quidditch star and Triwizard Champion ask her out as a date.  Then when she does get there she can't ignore the looks that people are giving her, including Harry, the stunned disbelief look.  From this event alone her self-esteem and self-confidence of her appearance has been tremendously boosted.

At the end of GoF, I cannot see Hermione as a girl lacking self-confidence nor self-esteem in her looks.  She knows she is pretty and she can be beautiful if she wanted to be.
