Me:

I thought this was just part of their love/hate relationship. They 
may be in the same house, but they dont travel in the same circles, 
and therefor don't really have permission/liberty to use the more 
intimate first name. It's a formality and a sign of respect, and like 
a title, in the right setting, it can become a form of endearment.
