concentrate on the other one while being, essentially, X-rayed with

I hadn't thought about it in those terms, but I think you're right; it
must have been fairly disconcerting for Snape if he ever tried to use
Legilimency on fake!Moody - direct eye-to-eye contact with that
swerving, swirling magical eye could not have been fun. In any sense
of the word.

As for "spots that don't come off," I'm not sure what Barty meant,
exactly. Once a Death Eater, always a Death Eater?

I always read that in a very direct fashion, especially given the line
where Snape clutched at his left forearm: I think Barty was referring
to the Dark Mark all Death Eaters carry, and pointing out to Snape
that he may have been Dumbledore's man, lo, these many years, but he
is still walking around with Voldemart's mark on his flesh, and that
that mark is a spot (on his character? his soul? or just simply his
flesh) that doesn't come off.
